SIPRANDA CAPITAL EMPLOYEE CONSENT FORM

Sipranda Capital Limited will process personal data. The employer may transfer such data to third-party service proviers such as insurance companies, insurance brokers, health facilities, provident funds, auditors, and information systems support providers.

The purpose of the processing of the personal data is for the performance of employment contract.

The legal basis for the Employer’s handling and transfer of personal data is:

1. For the performance of the employment contract
2. For compliance with legal obligations to which the Employer is subject to such as NHIF and NSSF.
3. For the legitimate interest of the Employer; and
4. To protect my vital interests.

In compliance with the Data Protection Act (DPA) 2019, and its Data Protection (General) Regulations, 2021, I therefore allow the Employer and its affiliates to collect and process my personal data in relation to the performance of my employment contract.

As such, I agree and authorize the Employer to:

1. Use and/or continue to use my personal data and information to process administer the statutory obligations arising from the employment contract.
2. Retain my information for a period of 10 years from the date of termination of the employment contract, or at such time that I submit to the Employer a written cancellation of this consent, whichever is earlier. I agree that my information will be deleted/destroyed after this period.
3. Retain information in the Employer’s database shared with other affiliates of the Company in accordance with the Data Protection Policy.
4. Share my information to affiliates and necessary third parties for any legitimate business purpose. I am assured that security systems are employed to protect my information; and
5. Inform me of future use and its offer using the personal information I shared with the Employer.
6. Recover and remit on a check off basis any amounts advanced to me by SIPRANDA CAPITAL including all applicable fees (facilitation fee, applicable taxes, and processing fee of 10/-per transaction)per monthly payroll cycle.
7. Discontinue my check-off if I choose to opt out of the service through the Application
